Headings. Each heading (H1 through H6) starts a new card and becomes its front; the paragraphs, lists, and images beneath it — up to the next heading — become the back. If your notes have no headings, add them where each card should begin, or you'll get one large card instead of many.
Yes. In Google Docs choose File → Download → Microsoft Word (.docx), then upload that file. Docs export cleanly to .docx, so the same heading-to-card behaviour applies.
The most common cause is a file renamed from another format rather than exported as a real .docx — for example a .pages or old .doc file given a .docx extension. Open it in Word or Google Docs and re-save or re-download as .docx, then upload again.
Bold, italic, bullet and numbered lists, and embedded images all come across. Images are bundled into the deck so they render on the card rather than linking out. Equations stored as text need MathJax enabled in your Anki template to display.
