Only if the scan has a text layer (OCR). Most modern textbooks and slide exports do. A photo of a page with no text layer can still become image cards: 2anki asks whether two pages make one card or each page makes its own. For text cards, run it through an OCR tool first — macOS Preview and Adobe Acrobat both do this — then turn on "Extract text from PDFs" in the card options.
A short line followed by longer text counts as a heading. Each heading becomes a card front and the text under it becomes the back. When a PDF has little text per page, like many slide exports, 2anki asks whether to make image cards instead; turn on "Extract text from PDFs" in the card options to make text cards from it. You can edit the cards in Anki after — we don't lock anything down.
Yes, but big PDFs take longer and can create huge decks. We recommend uploading one chapter at a time — easier to review, easier to share, and Anki handles 500-card decks better than 50,000-card ones.
Diagrams come across as embedded images. Equations rendered as images stay images; equations stored as text need MathJax enabled in your Anki card template to display.
