Either works. .csv is the simplest — plain text, one row per card. Choose .xlsx if your sheet already has the columns arranged the way you want and you'd rather keep the spreadsheet structure. Both land as the same deck.
Yes. After upload you map columns to the card's front and back, so it doesn't matter which order they sit in your sheet. Extra columns can be left out or kept as notes.
